This registration can be cancelled without any prejudice until final registration at the end of 2005.

On the basis of the information and conditions given in the 20.08.2004 document, I hereby request the registration of the following persons for participation: (surname, first name and full address)

The persons registered will take part in the following program(s)

5-day pre-program in the form of a sightseeing tour plus participation in the UNICA 2006 Congress in Shilla, South Korea, according to the description provided in part 2 of the document dated 20.08.2004 

exclusively participation in the UNICA 2006 Congress from 26.08.2006 to 03.09.2006, accommodation in the top-class 5 Star CONCORDE Hotel in Shilla, including the congress card and hotel accommodation (with breakfast).

I will make all necessary arrangements for transport to Seoul with a travel agency and I will provide relevant data concerning the airline selected and the time of arrival as soon as I have them.

You will need a valid passport for this trip. Please get relevant information from your travel agency or the competent authorities in your home country. 

The Korean organiser has given assurances that French, English and German speaking guides will be in charge of participants from the very beginning of the tour.
